Issuance of Admission Slip for Radio Operator Examination

1. Duly accomplished APPLICATION FOR RADIO OPERATOR EXAMINATION [Form No. NTC 1-01]

      1. Birth Certificate
      2. Baptismal Certificate
      3. Passport
      4. PRC License
      5. Driver’s License OR
      6. any document which can serve as the basis for age requirement

*Note 1: The applicant has to show the Original.

  2. Proof of attendance of seminar conducted by NTC accredited Amateur Radio Club NTC
  3. ID picture (1” x 1”) taken within the last six (6) months [Two (2) pcs.]
  4. Mailing envelopes with stamps (4 1/8” x 9 ½”) [Two (2) pcs.]
  5. IF for upgrade to higher class, Copy of valid Amateur Radio Operator Certificate issued by the National Telecommunications Commission.
